What To Expect

Welcome to Cornerstone Family Dentistry!

We look forward to becoming your dental provider and serving you through the years. For your first visit, we ask that you please arrive 10-15 minutes early to fill out necessary paperwork. You can also visit our Patient Forms page on our website to print our forms and get a head start on filling them out. Please remember to bring them to your first appointment. These forms will help our dental team become familiar with your medical and dental history and help us know how to best meet your individual needs.

During your Visit

During your first visit to our practice you will be introduced to our talented dental hygienist, Tina. She will review your dental and medical history, assess your current oral health, take the necessary x-rays with our state-of-the-art digital sensors, and complete a thorough cleaning. After your cleaning, Dr. Durusky will discuss with you his recommended treatment plan, if necessary, and answer any questions you may have. We want you to feel confident when choosing your course of treatment.

If you are anxious or nervous prior to your first appointment, please inform our team. It is not uncommon for people to experience anxiety prior to a dental appointment, so we understand and will do everything within our power to make you feel comfortable. It may help if you take some time to review the “Our Team” page and familiarize yourself with Dr. Durusky and the team.

What to Bring

• Records and/or x-rays from previous dental providers
• Dental insurance card
• The co-payment required by your insurer – we take cash, check, credit/debit cards and CareCredit. 
• A list of the medications you take
• Patient forms, if you printed them and filled them out
